  • Includes medical, social, residential, educational, emotional, and other related needs
Assessment and History
  • Includes trauma or abuse, substance use, health/medications/self-eare, general capabilities
  • Residential supports, employment, and training
Service Planning
  • Includes goal setting, supports, identification of providers, unmet needs
  • Review of needs and services for 90, 180, and 270 days
Coordination of Services
  • Includes referrals, access/delivery/follow up, crisis management planning
  • Developing access to lease restrictive services as needed
Contact / Monitor / Advocate for Continuity of Care in the community setting with other professionals, caregivers, service providers, guardians/family
  • Provide information, consultation, and support
  • Assistance in developing communication for self-advocacy
